Collaboration can vary greatly in ghostwriting because some authors provide an outline and completed research, and other times it is up to the ghostwriter. In book doctoring, there is also a range. The writer does a massive job to rewrite and restructure a book, but the manuscript is already written. Sometimes a book doctor provides coaching and suggestions without doing the rewrite.
